I've just merged all new changes to the current live package.
Can you please:

1. Download the latest zip package from https://storejextensions.org/myproducts.html
2. Install it as usual
3. Use a new clean user to place a new order

The invoice format using placeholders {user_address} and {billing_address} is as visible in the screenshot below, including all fields for the address.
