about the first question, this is expected because the Screen Reader is a plugin integrated with the Joomla multilanguage and assumes the frontend language as the language of the text on the page, see this FAQ http://storejextensions.org/faq/may-i-use-mixed-language-contents-or-google-translate-with-the-screen-reader.html

Access keys can be customize under plugin settings if needed, on Windows/Chrome usually the combination is ALT+SHIFT+access key

The voice is served automatically by Google Translate speech, it can't be selected male/female.
