Unfortunately JSN Uniform uses ajax and to be integrated requires a special configuration.

You can do this using the following custom settings to include the checkbox in the form:

To make the checkbox a mandatory field for JSN Uniform, add the class 'checkbox-required' to the container element using the parameter 'Checkbox template':
